OSHA's lockout/tagout standard — 29 CFR 1910.147 — is not a suggestion or a guideline. It is a federal regulation with the force of law, enforced through inspections that can result in citations exceeding $15,000 per violation. Year after year, LOTO violations rank among OSHA's top ten most-cited standards — not because facilities ignore the regulation, but because the regulation requires a documented, machine-specific, audited program that many facilities create once and never update. This guide covers the six requirements of an OSHA-compliant LOTO program, the training categories, the periodic inspection protocol, and the audit process that keeps a program current as equipment, personnel, and procedures change.

The machine-specific LOTO procedure is the document that an authorized employee follows at the equipment. It is not a generic checklist — it is a step-by-step instruction specific to one machine or one group of identical machines. A well-written procedure includes a photograph or diagram of the machine with each energy isolation point labeled, matching the step numbers in the written sequence. The authorized employee should be able to walk up to the machine with the procedure in hand and physically identify every isolation point, every lockout device type required, and every verification step — without ambiguity and without needing to call a supervisor for clarification.
The procedure must cover the complete sequence: notification of affected employees, normal shutdown using the machine's standard stopping procedure, isolation of each energy source at its dedicated isolation device — not at a control panel push-button, which does not isolate energy — application of lockout devices at each isolation point with individual padlocks, release or restraint of stored energy (discharged capacitors, blocked raised loads, relieved hydraulic pressure, bled pneumatic lines, cooled thermal energy), and verification of isolation by attempting to start the machine using its normal operating controls — the attempt must fail, proving the machine cannot start — then returning controls to the OFF position before maintenance begins.
The most common OSHA LOTO citation after missing procedures is inadequate training documentation. Employers must train three categories of employees and document the training for each: authorized employees — those who physically apply lockout devices and perform maintenance — require comprehensive training covering energy types, hazard recognition, procedure execution, device application, group lockout protocol, and the specific LOTO procedures for every machine they service. Affected employees — those who operate machines being locked out or work in the area — must understand the purpose of LOTO, must recognize a lockout when they see one, and must know never to attempt to start locked-out equipment or remove lockout devices. Other employees — those who occasionally enter LOTO areas — require awareness-level instruction on recognizing lockout and not interfering.
Retraining is required when: the LOTO procedure changes, the equipment or energy isolation points are modified, a periodic inspection reveals that an authorized employee does not understand the procedure, or the employer has reason to believe there are deviations from the procedure. Retraining must be documented, dated, and retained.
OSHA requires that each machine-specific LOTO procedure be inspected at least annually by an authorized employee — but not the authorized employee who normally uses that procedure. An independent inspector reviews the procedure against the actual equipment: verifying every isolation point still exists and is accessible, confirming the lockout devices specified still fit the isolation points, observing an authorized employee perform the procedure to identify deviations or misunderstandings, and certifying the inspection in writing with the equipment identification, date, employee names included, and inspector name.
The periodic inspection is where theoretical compliance meets practical reality. A machine that was modified six months ago may have a new isolation point not documented in the procedure. A lockout device that was adequate when purchased may have been superseded by a newer model that fits better. An authorized employee may have developed a shortcut that bypasses a step — a shortcut revealed by observation during the inspection. The annual inspection closes these gaps before they cause an incident or an OSHA citation.
